Usually a defrost problem rather than a refrigerant one — a failed timer, heater or termination sensor. Chipping the ice away clears the symptom for a day; finding out why it formed is the actual repair.
Condenser coils choked
Kitchen grease and dust build a blanket over the coil, and the system loses its ability to reject heat. It is the single most preventable failure in commercial refrigeration and the one most often left until something stops.
